Long Term Condition Reviews – FAQs:
- What to do if it’s just been my birthday and I’m not due my long-term review until September?
Patients are recalled around their birth month, receiving three invites leading up to that time. If your birthday has just passed and your review was scheduled for later, you will still be recalled in line with your birth month in the future.
- With reference to your updated plans for the annual long-term review, I am not usually in the country during my birth month. How would this be accommodated?
If a patient cannot attend during their birth month, they still receive three invitations over a three-month period. If they are away for all of these, they would need to arrange an alternative appointment upon their return.
- What happens if my birth month has already passed for the long-term condition?
If your birth month has passed, you will be included in the next recall cycle based on your birth month. The system ensures that patients are recalled annually.
- Hi, I am not generally in the country around my birthday, how would this be accommodated?
You would still receive the standard three invitations leading up to your birth month. If you are away for all of these, you would need to book an appointment outside of the recall process when you return.

We’ve introduced a new system to make managing your Long-Term Condition (LTC) reviews simpler and more flexible. Your annual review will now align with your birth month, making it easier to remember and plan ahead. You’ll receive up to three invitations, starting up to three months before your birth month, giving you plenty of time to book at a time that suits you.
This change means you may receive your first invitation earlier than expected—this is simply to bring you in line with the new schedule. If your review includes a blood test, this will be arranged in advance so everything is ready for your appointment.
By introducing this system, we’re making it easier for you to stay on top of your health with more flexible appointment options, better continuity of care, and a simpler recall process. If you’re away during your birth month or need to book at a different time, there’s now a wider window to find a convenient appointment.
Thank you for being part of this improved process—we look forward to seeing you soon!
